Our home care agency is seeking a compassionate and detail-oriented Registered Nurse (RN) to join our team. In this role, you will create and manage individualized care plans, ensuring the highest quality of care for clients in the comfort of their homes. Responsibilities
- Develop, implement, and evaluate personalized care plans in collaboration with patients, families, and healthcare providers.
- Administer medications, treatments, and injections as prescribed by physicians.
- Monitor and document patient health conditions, vital signs, and progress accurately.
- Report significant changes in condition to physicians and supervisors promptly.
- Educate patients and families on disease management, home safety, and proper care techniques.
- Perform wound care, catheter care, and other skilled nursing procedures as needed.
- Ensure infection control practices and safety protocols are followed in the home setting.
- Supervise and provide guidance to caregivers, CNAs, and other direct care staff.
- Maintain accurate and timely clinical documentation in compliance with state, federal, and agency standards.
- Support patients and families in managing chronic conditions and coordinating follow-up care.
- Participate in agency quality improvement initiatives and staff meetings as required.
- Respond to emergencies and provide appropriate interventions to ensure patient safety.
Qualifications & Requirements
- Active Registered Nurse (RN) license in the state of employment.
- Current First Aid and CPR certification.
- Negative TB test (required).
- Valid driver’s license and reliable transportation.
- Previous nursing experience preferred, especially in home care or community settings.
- Strong interpersonal, organizational, and client care skills.
- Ability to work independently and as part of a collaborative care team.
Compensation: $50.00 per hour
